Dental Plan Referendum Question

Do you support the University Students’ Council offering dental coverage in accordance with the following:

1)  The dental coverage would initially cost $114.00 per year (for 12 months of coverage), and would come into effect September 1, 2012.  Students could add their family (spouse and/or dependants) for an additional fee of $171.00.

2)  The fee would be adjusted annually according to the Consumer Price Index for Ontario to reflect inflation.

3)  Students with or without dental coverage could opt-out of the dental plan during the first 30 days of September, and be refunded for the entire cost of coverage.

4)  The Dental benefit would initially provide coverage for:
-      100% diagnostic and preventative services
-      80% fillings and minor restorative
-      80% anesthesia
-      80% periodontal scaling
-      80% removal of impacted wisdom teeth
-      10% root canal treatment
-      10% periodontics (other than scaling)
-      $750 annual maximum

5)  Students covered with the dental benefit plan would be able to see the licensed dental practitioner of their choice in Ontario.
